Abstract

The study included 128 cases; 64 SLE cases and 60 healthy controls to evaluate autonomic nervous system affection in SLE in addition to peripheral nervous system affection and its relation with autonomic affection.
According to our knowledge, this is the only study evaluating the autonomic function and peripheral nervous system in SLE in Upper Egypt. We think, our study group reflects the degree of dysautonomia and peripheral neuropathy in SLE in the patients attending to our hospital from the various parts of upper Egypt.
To conclude, in this study we found that SLE patients there is increase in possibility of peripheral nervous system affection either autonomic or peripheral.
This finding may give us a novel insight in identification of SLE either associated, attributed or drug induced neurological affection with just Ewing battery method and NCS and so using the proper method of treatment.
In our study and according to Ewing battery and SSR, it was found that there is affection in autonomic function assessment tests in SLE patients. Also it was found that there was no significant relationship between autonomic affection and BMI, age and duration of the disease.
Peripheral neuropathy was demonstrated in SLE patients in comparison with healthy controls, more predominant in LLs especially peroneal nerve, axonal affection. Motor affection was more prevelant than sensory affection.
As we concluded, there was no significant relationship between autonomic and peripheral neuropathy in the studied population.
Recommedations:
- We recommend that more studies with larger numbers must be done to find the exact relationship between SLE and autonomic and peripheral neuropathy prevelance.
- We recommend that more studied must be done for assessment of cardiac dysautonomia in SLE patients with use of other tests in addition to Ewing battery for better assessment, especially that cardiac dysautonomia is one of the common causes for sudden death in SLE patients. Especially that some drugs prescriped in SLE have cardiovascular effects.
- We recommend that disease activity must be put in mind, with further studying of its relationship with autonomic affection, which unfortunately is deficient in our study.
- Also, it is recommended to study the effect of the prescriped drugs on autonomic nervous system and whether it is dose related or not in order to adjust the effective treatment.
- We recommend to assume follow up protocols for SLE patients to control affection of the nervous system.
