Abstract

Background: Budd-Chiari Syndrome (BCS) is a spectrum of disease states, including anatomic abnormalities and hypercoagulable disorders, resulting in hepatic venous outflow occlusion from the small hepatic veins up to where the inferior vena cava enters the right atrium. According to the etiology, BCS can be classified as primary (due to intrinsic intraluminal thrombosis or webs) or secondary (due to intraluminal invasion by a parasite or malignant tumor or extraluminal compression by an abscess, cyst or solid tumor).
Aim: The specific objective of this study is: To identify and compare the predictive ability of prognostic indices in Primary Budd Chiari Syndrome Egyptian patients regarding the following items: One year survival rate on different lines of treatment (medical, angioplasty with and without stenting and TIPS and Patency and rate of occlusion of applied stents following radiological intervention (angioplasty and TIPS) during one year of follow up.
Methodology: This study was conducted at Tropical Medicine Department Ain Shams University Hospitals. A combined retrospective and prospective cohort study was carried out. We retrospectively analyzed the available data base of all patients diagnosed between 2005 and 2013 and prospectively for those patients who were diagnosed between the beginning and the end of 2014.
Results: The current study was conducted on 194 consecutive Egyptian patients with confirmed diagnosis of BCS who were admitted to Ain Shams University Hospitals, Tropical Medicine Department or attending the outpatient clinics. The patients were affiliated to the Budd Chiari Study group (BCSG).
Conclusions: Primary BCS in Egyptian patients develops at younger age than their matches in Europe and Asia and IVC thrombosis and/or PVT are related to poor prognosis in BCS Egyptian patients.
Recommendations: Future studies with more included patients and extended follow up durations are eagerly needed to find out more independent predictors of survival in Egyptian BCS patients. Multidisciplinary team approach as well as proper selection of patients are recommended for better outcome of BCS patients.
