Abstract

Endometrial cancer is the most common gynaecological cancer in developed countries, and its incidence is increasing. The current study is a prospective cross sectional study that aimed to compare preoperative staging of endometrial cancer using MRI with surgical staging. The study included 58 consecutive patients with pathologically proved endometrial cancer attending the outpatient clinic of Obstetrics and Gynecology department at Mansours University Hospital during a period between December 2012 and January 2016. Main outcome results: Our results showed that: for prediction of deep myometrial invasion, MRI had sensitivity of 96% and specificity of 90.91% with 88.89% PPV and 96.77% NPV, and 93.1% diagnostic accuracy. For prediction of cervical involvement, the sensitivity was 80% and the specificity was 100% with 100% PPV and 98.15% NPV, and 98.28% diagnostic accuracy. The MRI had 60% sensitivity, 98.11% specificity, 75% PPV and 96.3% NPV, and 94.83% diagnostic accuracy for prediction of LNs metastasis. Conclusion, MRI had a high diagnostic accuracy in predicting deep myometrial invasion, cervical invasion, and LNs metastasis, and is a valuable imaging modality for presurgical staging of patients with endometrial cancer.
