Abstract

Periprosthetic fracture is a serious complication around hip arthroplasty that can be difficult to treat and can be potentially complicated.
In our study 20 patients ranging from 35 years to 80 years presented with periprosthetic fracture. 7 were females (35%) and 13 were males (65%).
Six patients (30%) were suffering from intraoperative fracture and fourteen patients (70%) were suffering from postoperative fracture. One case (5%) was acetabular periprosthetic fracture while nineteen cases (95%) were femoral periprosthetic fracture.
We used different modalities in treating such complication:
A- In acetabularperiprosthetic fracture we used:
o Internal osteosynthesis through cup in one case(5%).
B- In femoral periprosthetic fracture we used:
o Cerclage wiring in 3 cases (15%).
o Internal fixation in 6 cases (30%), two DCP (10%), one Mennen plate (5%), two cable-plate (10%) and one locked plate (5%).
o Revision anthroplasty using long stem femoral prosthesis in 9 cases (45%), 5 reef (25%) and three long stem cementless unlocked (15%) and one case long stem cemented (5%).
o Supracondylar nail in one case (5%) for treatment of type C fracture.
According to Harris hip scoring system for functional evaluation of our results, twelve patients had excellent results, five patients had good results, one patients had fair results and two patients had poor results.
Periprosthetic fracture around a hip arthroplasty are not uncommon complication.
 Our study include 20 patients if different types of fracture (according to Vancouver classification) and were treated with different methods. So, we will discuss the results of our work in comparison with other many author’s studies that nearly resembles our work in trial to obtain the best choice of treatment for each type of fracture.
Periprosthetic femoral fracture:
The following algorithm can summarize the different methods used in treatment of periprosthetic femoral fracture occurred postoperatively.
