Abstract

Temporomandibular joint dysfunction is a group of abnormalities.Pain and limitation of jaw movements can greatly affect the patientinterfereing with work and social life.The incidence of TMJ dysfunction is about 39% worldwide 3:1 female to male ratio.
Temporomandibular joint dysfunction is classified as internal derangement, osteoarthritis, and muscle spasm.
Imaging modalities ranging from plain x ray, ultrasound up to CT and MRI imaging.Tmj dysfunction as osteoarthritis and internal derangement mostly detected by MRI, which considerd the most simple non invasive way in diagnosis of tmj dysfunction. CT is very effective in diagnosis of ankylosis.
The good choice of the type of imaging has an important role in proper diagnosis, hence suitable treatment.
The treatment plan of management differs from patient to another one and depends on type of the pathology, severity and time of intervention. Latency of treatment will take us to the more difficult and recurrent modalities. The medical treatment is considered the most common modality of treatment.
Improvement of pain and clicking and increasing the range of motion of jaw movement is very important, it is considered the main aim. It greatly affects and alters the patient life and makes it better. Daily activities, like speaking, eating and work are very important to the patient to be imporved.
Postoperative physiotherapy is very important for the patient for good jaw movement and prevention of ankylosis occurrence.
Postoperative complications such as, recurrence of pain orreankylosismay occur and must be treated .
Conclusion
Temporomandibularjoint dysfunction is a group of TMJ abnormalities. Including internal derangement, arthritis, ankylosis.
Although it is common worldwide, in our locality it is under estimated due to lack of knowledge and education and not all patients are seeking medical advice.
 Imaging of the TMJ pathology guided us for proper and correct management. There are a wide variety of imaging modalities for the temporomandibular joint dysfunctions.MRI is considered the gold standard imaging tool for diagnosis of the dysfunction in a simple and perfect way without intervention. It gives superior information about joint structures, including disc morphology and position. Effusion, bone marrow edema, and osteoarthritis also detected. CT imaging is most helpful in diagnosis of bony lesions, ankylosis, and neoplasm.
Treatment oftemporomandibular joint expresses a very wide variety of modalities, ranging from the self care and rest up to the complicated surgical procedures as joint replacement
 Medical treatment considered the most common method in treatment and gives a suitable result for a big number of patients.
 Splint therapy also used with medical treatment in patient for relief of joint stress and tension and effective tool as night guard(bite blockade).
 Artheocentesisis considered the simple interventional modality which is very effective in improving pain and jaw movement in patient resistant to medical treatment.
 Arthoscopylysis and lavage and diagnosis of joint abnormalities has greatly a beneficial role in management.
 Discectomy, disc repositioning, conylectomy, and arthroplastyas a surgical alternatives. Patients almost always may need more than one surgical modality.
 Joint replacement used with severe and recurrent joint abnormalities, considered difficult in our locality due to its high cost.
