Abstract

End-stage renal disease (ESRD) has become a public health concern
worldwide as the total number of ESRD patients requiring renal replacement
therapy has been growing dramatically.
One of the most important complications of ESRD is renal bone disease
(renal osteodystrophy).
The present study was aimed to study the prevalence of MBD among
hemodialysis patients in Menoufia governorate-Egypt.
The study included 100 patients on regular hemodialysis for ≥ 3 months.
They were randomly selected from four governorate dialysis centers during the
period from November 2014 to August 2015.
The diagnosis of MBD was made according to the presence of MBDrelated
symptoms, radiological and laboratory data.
According to the levels of serum PTH, the included patients were classified into
3 groups:
• group I: patients with PTH level <150 pg/ml.
• group II: patients with PTH level 150-300 pg/ml.
• group III: patients with PTH level > 300 pg/ml.
Results showed:
• Hyperparathyroid bone disease was prevalent in 55% of the studied
patients and low bone turn-over in 30% of the studied patients, and both
of the 2 groups showed highly significant association with serum calcium
and phosphate levels.
• Suggestive MBD related symptoms were prevalent in both high bone
turnover and low bone turnover ( 60% & 70% respectively).
• Hyperparathyroid group showed 46 patients (83.6%) with hypocalcemia
(serum calcium levels <8.5 mg/dl).
• Hypocalcaemia and hyperphosphataemia were observed in 64% and 74%
of the studied patients respectively.
• Positive significant correlation between PTH and ALP, blood urea, URR,
Kt/V and serum uric acid, and negative significant correlation was
observed between PTH and serum creatinine, and between serum calcium
and blood urea & serum uric acid.
• According to radiological examination of 22 patients of the included 100
patients, renal bone disease was prevalent in 68.2% of patients (15/22).
