ABSTRACT Background: chronic obstructive pulmonary disease (COPD) is a common entity in clinical practice. Development of right ventricular hypertrophy and eventual right side heart failure is also common in such patients. However, some disturbance in left ventricular (LV) function has been observed among such patients. Objective: The aim of this study was to assess the cardiac changes secondary to chronic obstructive pulmonary disease (COPD) and to evaluate LV systolic and diastolic function by echocardiography in critically ill COPD patients. Results: LVDD among COPD patients was (88.6%) compared to control group (24%) . The risk ratio of diastolic dysfunction in critically ill COPD patients was 24.6. It was found that RVSP had a sensitivity of (83.9%) and specificity of (85.3%) at a cut off value (31mmHg) predicted with overall accuracy (76.5%) in predicting LV diastolic dysfunction among COPD patients. There was no significant differences in parameters assessing the LV systolic function between both groups. Conclusions: chronic obstructive pulmonary disease patients have a high prevalence of left ventricular diastolic dysfunction, which is associated with disease severity. Because of this association, it is important to exclude decompensated heart failure during chronic obstructive pulmonary disease exacerbation.
