Abstract

The aim of this study is to study small bowel changes in egyptian patients with liver cirrhosis and portal hypertension
This study was conducted on seventy individuals including forty known cirrhotic patients and thirty non cirrhotic patients as a control group did not suffer from any chronic liver illness, but were indicated for small bowel capsule endoscopy or enteroscopy.They were selected from out patient clinic and inpatient of Tropical medicine department , Minia university hospital and from Gastroentrology and Endoscopy Unit , Wadi Elneel hospital.
The cirrhotic patients were divided into two groups:
group I : known cirrhotic patients without esophageal or gastric varices their number were twenty (20)
group II: known cirrhotic patients with esophageal or gastric varices , their number were twenty (20)
All patients were subjected to the followings:
Detailed history and thorough clinical examination
Laboratory investigations
Abdominal ultrasonography
Upper GIT endoscopy
In each group;small bowel capsule endoscopy or single ballon enteroscopy have been done
We have found small bowel lesions in the form of mucosal inflammatory lesions and vascular lesions in about 57.5% of cirrhotic patients versus 6.7% of control patients. In group I patients the prevalence of small bowel changes was 55% versus 60% in group II with no significant statistical difference between the two cirrhotic groups in the presence of small bowel changes.
We have found that Capsule endoscopy was better than antegrade single ballon enteroscopy in the detection of small bowel changes due to portal hypertensive enteropathy.
We have found that small bowel changes were related to the presence of portal hypertensive gastropathy and more in those with advanced chronic liver disease child class B or C and in those with higher MELD score, and the small bowel changes were present in those with higher degrees of anaemia.
We studied the effect of different variables including age, sex, Child score, MELD score, portal hypertensive gastropathy and esophageal, gastric varices on the presence of portal hypertensive enteropathy by univariate and multivariate analysis
By univariate analysis we found that neither patient age nor sex was related to the presence of small bowel changes.
Patients with advanced chronic liver disease Child class B or C has a positive effect on the presence of small bowel changes, high MELD score has a positive effect on the presence of small bowel changes.
