Abstract Fol lowing major arterial surgery the vascul ar patients are subject to specific effects that affect the body both general ly and systemical ly which is of paramount importance as the success of an operation depends on the knowledge of the vascul ar surgeon by these effects and how to deal with to correct any aberration. The general and systemic body effects shortly includes: • Dramatic decrease in cardiac output and arterial pressure. • Impairment of the oxygen del ivery to the tissues. • serious coagul ation defects. • Left ventricu l ar diletation. cardiac ischemia and heart fai lure. • Severe hyp.,rtension wi th subsequent hemorrhage cllld neurol ogical deficits. cerebra l • Decrease of the bl ood flow to the viscera, kidneys and spinal cord with subsequent co lonic ischemia, renal failure, and irreversibl e ischemic cord damage. • Significant metabol ic acidosis and electrolyte abnorma l ities. The purpose of this work is to further stress the need for proper pre, intra, and postoperative monitoring which includes : .. Perioperative pool scan. electrocardiogram and gated blood * Routine laboratory data including urine analysis, complete bl ood count, serum electrolytes, blood urea nitrogen, creatinine l evels, platelet count and prothrombine and partial thromoblastine time. * Invasive monitoring of cardiac function as arterial cannulation, central venous pressure l ine, and Swan-Ganz catheter. * Arteriograms,ultra sound, computerized tomograms and Doppler flow detector.
