Abstract

Diabetes mellitus and hypertension, as a diseases with a high prevalence of growth in all countries, threatens to become a global epidemic risk, and thus the question of the ability to work of this category of patients is extremely important in terms of professional orientation, professional selection, work absenteeism and disability evaluation The objectives of the study were assessing the effect of Diabetes mellitus and Hypertension on work productivity. The study was case control nested in a cross sectional study. The study was conducted in the context of time frame (from the beginning of April of 2015 to March 2016) The study was done in two sites at one in Urban and other in Rural. As regarding, The Urban area was one of the primary health care center of Shebin EL-Kom city, Shebin EL-Kom district, Menoufia governorate and The Rural area was the primary health care center of Batanon village, Shebin EL-Kom district. Menoufia governorate. Study population were all HTN, DM patients and their relatives attending to primary health care center in between May 2015 to October 2015 duration of six months. The recruited participants were 800 participants (400patients and 400 control). They were enrolled from an urban and a rural family health units as follow: 1) Urban area (Sheben Elkom city) 218 patients were participated to be the target of the study (93 hypertensive &125 diabetic). 2) Rural area (Batanon Village) 182 patients were participated to be the target of the study (84 hypertensive and 98 diabetic).
3) A similar numbers 400 control participates were recruited for the study. The cases were reached through the attendants coming to the family health center seeking treatment , follow up or coming with a patient .All the attendants were subjected to an interview .The interview has the following aims to discussion of the study aims and designs, taking consent. Filling the questionnaire with the patient own words and registering their answers in the questionnaire form. Questionnaires are: Sociodemographic questionnaire was used to collect current data regarding the respondents’ characteristics such as sex, age, place of residence, marital status ,education, occupation and disease questions were duration of diabetes mellitus and hypertension ,treatment ,number of working days ,numbers of absenteeism days ,numbers of presenteeism days ,number of working hours and work productivity assessment will be performed for all patients including: HRQOL ,WPGH and Job satisfaction questionnaires In presented study. Work absenteeism, work productivity loss and impairment were more with the diabetic patients than the hypertensive patients (P<0.000).There was statistically significant difference subscales between the studied groups in all sub scales of HRQOL items except physical functioning . For the mean scores of HRQOL for studied group that The highest HRQOL score was scored by healthy group then the second highest HRQOL score was scored by diabetic patients where the lowest HRQOL score was scored by hypertensive patients. There was statistically significant difference between studied groups and job satisfaction where below average job satisfaction score was scored by the diabetic patients while the average job satisfaction score was scored by hypertensive patient. There was positive correlation among the studied groups regarding job satisfaction score and work productivity score. There was statisticaly significant factors effecting quality of life of the diabetic patients as follows: Duration of illness, Socioeconomic, Age, Job and Family Size Also, there was statistical significant factors effecting Factors of the quality of life for the Hypertensive patients as follows: Socioeconomic, Duration of illness, Job, Age and Sex . There is a need to develop and implement effective targeted intervention to help workers to manage their disease better.
