Abstract

The distal femoral fractures are those fractures that involve the distal nine centimeters of the femur; they represent a real challenge as regard the method of treatment. In the current study twenty adult patients with fractures of the distal femur types A and C were surgically treated by LISS distal femoral plate and followed up. The age incidence ranged from 45 years to 72 years, with a mean of 58.45±7.44 years, 8 males were affected while 12 females were equally affected. The right side was affected in 11 patients (55%) and the left in 9 patients (45%). 8 patients were fractured due to direct fall on the affected limb (40.0%), 9 patients were injured due to Pedestrian traffic accident (45.0%) , 2 patients were injured due to Motor cycle accident( 10.0%) and one due to twisting injury (5.0%).There were 5 cases with type A1 fracture, 4 cases with type A2 fractures, 3 cases with type A3 fractures, 3 cases with type C1 fracture, 3 case with type C2 fracture and 2 cases with type C3 fractures according to the OTA/AO classification. All patients were followed up for a mean follow up period of 5.60±0.50 months and studied both clinically and radiologically according to the functional evaluation scale developed by Sanders et al for distal femoral fractures as regards: range of motion of the knee joint, deformation, pain, walking ability and return to work. The results were excellent in 13 cases (65%), good in 5 cases (25%) and fair in 2 cases(10%).
The complications encountered in this study were superficial infection, loss of knee flexion by about 10º-20º, not full union, and loss of length less than 0.5 inch compared to the sound limb.
