Abstract

Endoscopic nasal surgery is one of the most frequently performed procedures in ENT practice. The causes of treatment failure of endoscopic nasal surgery include disturbed mucociliary clearance, immunodeficiency, sensitivity to acetylsalicylic acid and anatomic obstruction .Synechiae and stenosis formation following endoscopic nasal surgery represents a potential cause of surgical failure. MMC has been used successfully in other fields to decrease post-operative scar formation. MMC has been used more recently in ENT procedures such as laryngotracheal stenosis repairs, to maintain patency in myringotomy, and its use in sinus surgery has been successfully studied.
The aim of the study:
To assess role of MMC in prevention of synechia formation and ostia stenosis following endoscopic nasal surgery.
Patient and Methods:
At completion of endoscopic nasal surgery in 30 patients. A cotton pledget saturated with 2 cm of 0.5 mg/ml MMC was placed for 5 minutes in right side of nose at site of operation (In middle meatus or between the turbinate and the septum) and similar saline soaked pledget was placed on the opposite side. Patients were examined postoperatively for the presence of synechiae, size of middle meatal antrostomy (in FESS group only) and subjective evaluation was done with follow up period up to 2 months.
