Abstract

Neonatal deaths account for 40% of all deaths in children under-five. There is a dramatic survival gap for premature babies depending on where they are born. The availability of neonatal intensive care has improved outcomes for high-risk infants including those born preterm or with serious medical or surgical conditions.
Newborn networks have been set up to achieve service rationalization, improvement of quality of care and help to gain clinical engagement at a very early stage .
EGNN aims to train pediatricians on electronic registration of neonatal health records, to develop the skills of clinicians to achieve improved outcomes through the improved quality of delivery of health care, to establish and maintain a neonatal database that will allow comparison between participating hospitals, to study the mortality and morbidity of high risk infants admitted to the participating neonatal care units, to be a basis for policy planning and resource allocation, quality improvement and management at a departmental, national and regional level and to join other international neonatal networks, which will provide the ability to benchmark healthcare in Egypt against international standards.
This was a prospective study of all cases admitted to Neonatal intensive care unit in Menoufia university Hospital and Manshia soltan Hospital using the Egyptian neonatal network including all neonates admitted along 1 year from the first of March 2014 till the 28th of February 2015.
The data includes patient’s admission data (e.g. demographic data, neonatal measurements and maternal history), NICU stay data (e.g. average duration of stay, requirements for assisted ventilation, surgical interference, and development of complications) and discharge data (e.g. clinical outcome).

The total number of cases admitted to NICU of Menoufia university Hospital was 191 neonates and in Manshia soltan Hospital was 200 neonates.
In Menoufia university Hospital, 106 neonates were males while 84 were females and one case with ambiguous genitalia. 41.8% were born by normal vaginal delivery and 58.2% were born by CS. Also, only 2.1% of cases needed aggressive resuscitation with cardiac compression and 1 case needed drugs.
In Manshia soltan Hospital, 117 neonates were males while 83 were females. 51% were born by normal vaginal delivery and 49% were born by CS .
There was no significant relation between cases as regard gender and mode of delivery and clinical outcome in both hospitals .
In Menoufia university Hospital, The most common cause of admission was prematurity (66 cases) followed by neonatal jaundice (46 cases) while In Manshia soltan Hospital, The most common cause of admission was neonatal jaundice (88 cases) followed by respiratory causes (56 cases).
The most common complications among admitted cases were pneumothorax and chronic lung disease (CLD). pneumothorax occurred at a rate of (12.6%) In Menoufia university hospital and was significantly releated to prematurity, central venous line insertion and ventilation duration. and (8%) In Manshia Soltan hospital significantly releated to prematurity, central venous line and no significance with ventilation duration. CLD occurred at a rate of (13%) In Menoufia university hospital and (6.5%) in Manshia Soltan hospital.
Sepsis was one of the commonest morbidities in our study In Menoufia university Hospital which occurred in 18.8% of cases and was significantly related to prematurity, pneumothorax, duration of hospital admission > 7 days, duration of ventilation >9 days, central venous line insertion and blood transfusion In Menoufia university Hospital,while these parameters has no significance in Manshia soltan Hospital where sepsis rate was 14 %.

The most common result of blood cultures In Menoufia university Hospital was no growth (81.1%).
As regard clinical outcome, mortality rate was 31.9% In Menoufia university Hospital and 19.5% In Manshia soltan Hospital. the most common cause of death was sepsis followed by respiratory failure in both hospitals .
There was a highly significance in mortality with prematurity, sepsis ,pneumothorax, blood transfusion,central venous line, APGAR score <7 , and duration of hospital admission in both hospitals.
The most common birth defect was Arnold chiari malformation 3 cases In Menoufia university Hospital.
Phototherapy alone was the primary management in all cases of neonatal jaundice supported by exchange transfusion in only 2 cases in each hospital.
