Abstract

 Autogenous arteriovenous (AV) access for hemodialysis has been shown to be superior to prosthetic graft or catheter access in terms of patient morbidity and mortality. In addition, the maintenance of autogenous AV access is less expensive than prosthetic conduits.
 This study designed to evaluate short and mid term patency of basilic to radial transposition in the forearm in patients in need for regular dialysis.
 When the cephalic vein in the forearm is not considered adequate for an autogenous AV access, the forearm basilic vein is alternative. Secondary to its posteromedial location in the forearm, a transposition is always required to provide safe access for hemodialysis. Possible sites of arterial inflow include the entire trunk of the radial or ulnar arteries or the brachial artery.
 The study was conducted over 20 patients at Nasser institute, their ages between 18 – 55 years old with basilic vein diameter more than 2 m.m and good radial pulse.
 We explored the basilic vein in the forearm through stepladder incisions over the vein under local anesthesia, then transposition of the basilic through the volar aspect of the forearm from its bed to the radial artery.
 The operation Considered Successful if thrill over basilic vein felt immediately Postoperative.
 All patients have been subjected to Duplex u/s to asses diameter of the basilic Vein Postopertive.
 1 ry Patency meaned in our study that Fistula of Patient remain thrilling For 6 months without needing for any intervention.
