Abstract

Hepatic hydrothorax is a devastating complication of end-stage liver disease, and in fact, is not that uncommon as previously thought. Its management is challenging and frequently associated with poor outcomes in most cases.
Hepatic hydrothorax (HH) is an example of a porous diaphragm syndrome. Portal hypertension results in the formation of ascitic fluid which moves across defects in the diaphragm and accumulates in the pleural space. Consequently, the treatment approach to HH consists of measures to reduce the formation of ascitic fluid, prevent the movement of ascitic fluid across the diaphragm, and drain or obliterate the pleural space.
The main stay of treatment is diuretics and salt restriction. Thoracentesis and TIPS are usually indicated in these patients.
Our study is arandamised controlled study which was conducted in the Giza chest hospital on inpatient and outpatient departmentsto compare between the effect of diuretic and thoracocentesis on pulmonary function test in hepatic patient with transudative pleural effusion. Thirty patients was divided into two groups:
Study group (1): consists of 15 patients who received Lasix.
Study group (2): consists of 15 patients who underwent thoracocentesis.
Results:
Lasix treated cases had better FVC compared to other group with statistically significant difference in between. FVC was improved with statistically significant difference in FVC & FEV1\FVC ratio in cases treated with Lasix. FVC was improved together with FEV1 with statistically significant difference in cases treated with thoracocentesis. Lasix treated cases had better percentage of improvement as regard FVC and FEV1/FVC compared to other group with statistically significant difference. There was statistically significant positive correlation between pulmonary function versus age in cases treated with thoracocentesis.
