Abstract

Portal vein thrombosis refers to the complete or partial obstruction of blood flow in the portal vein, due to the presence of a thrombus into the vessel lumen.
PVT is an important cause of non-cirrhotic pre-hepatic portal hypertension all over the world. It accounts for almost 30% of all variceal bleeds and is the leading cause of variceal bleeding in children.
Over the last few years, it has been increasingly diagnosed by the widespread use of ultrasound Doppler.
The prevalence of PVT in cirrhotic patients at evaluation or at the time of transplantation varies from 5% to 26 %.
This study was carried out on 50 patients with portal vein thrombosis to specify the nature of the thrombus whether malignant or benign.
According to the tumor markers, MRI, DWI and biopsy from adjacent focal lesions to the Portal vein thrombus, the cases were classified into benign cases (14 cases) and malignant cases (36 cases). In the present study, the specificity of color Doppler U/S for diagnosis of malignant PVT as regards the presence of intra-thrombal pulsatile flow was 100% with 66.7% sensitivity, while the sensitivity of CT was 91.7 % with specificity 100%.
The MDCT and Doppler in comparison to Biopsy and MRI diffusion were found to have a sensitivity of 91.7 % and (94 %) accuracy.
