Abstract

Pain management is a crucial component in treating pediatric patients. The palatal injection is among the most feared by patients and causes more pain than injections in less dense soft tissues of the mucosa. The aim of the present study was to demonstrate whether Articaine/HCl administered alone as a single buccal infiltration for extracting upper primary molars provided favorable palatal anesthesia as compared to buccal and palatal injection. This was done by assessing the pain reaction for patients during and after the extraction procedure.
The study included 120 healthy children aged 5-6 years. Children were selected from those attending the outpatient clinic, Pediatric Dentistry Department, Alexandria University. Each child had an upper primary molar indicated for extraction.
The extraction procedures were videotaped for further assessment of pain reaction by the SEM scale, and after the extraction procedure, each child reported his own experience using the Wong Baker pain scale.
The results showed that patients who extracted their upper primary molars using only buccal infiltration had less pain reaction in both pain scales than those who had both buccal and palatal injections, with a statistically significant difference.
Regarding the gender, males showed less pain reaction than females with statistical significance in both groups.
Regarding the type of molar extracted, most of the patients who extracted the upper first primary molar showed more pain reaction than those who extracted the second primary molars in both groups.
Regarding the type of dental visits and type of treatment offered in previous visits, showed statistical significance in control group only.
A strong positive correlation was found between the two scales used in the study.
