Abstract

1) Research recommendations
• This essay was a trial to highlight updates in forensic psychiatry. This field of psychiatry is complicated and need many researches to resolve unclear matters and those of controversy.
• This may be done by revising more updates in forensic psychiatry in books and on the internet, and how to use these updates to develop more reliable risk assessment tools and more effective management ways and by giving attention to different fields of forensic psychiatry which has been enumerated in first chapter.
• Also, researches depending on data taken from our forensic psychiatric units and analyzing it in correct way will give us more clear data about most common psychiatric disorders causing patients to commit crimes and level of dangerousness at which psychiatric patients in our society commit crimes. This may help us to put valid risk assessment tools and management ways which are compatible with our society and our culture.
2) Clinical recommendations
• Forensic psychiatry should be practiced in accordance with guidelines and ethical principles enunciated by the profession of psychiatry.
• Forensic psychiatrist should have effective risk assessment tools and effective management ways to be able to give good results in his job.
• Routine risk assessment for psychiatric patients should be applied in each patient sheet.
• Proper management can convert this patient who committed crime which he is irresponsible for it to useful person to himself, his family and all society.
• Valid risk assessment tools should be used.HCR_20 may be applicable risk assessment tools to be used in forensic psychiatric hospital in Egypt.
• Increasing applicable effective drugs for both acute and maintenance treatment have an important role in managing the psychiatric disorders and to eliminate aggression.
• Reliable psychotherapy programs will help patients to recognize and to overcome their problems.CBT and group therapy will help, also art therapy especially drama therapy helps patients to ventilate their feelings and to spend time in useful enjoyable way.
• Rehabilitation is very important and could be done in different ways to satisfy different patients, as different sports, playing music and drama psychotherapy. We can raise educational level of these patients by encouraging educated patients to teach those uneducated and putting rewards for these teachers. Also, there are institutions which can train patients on different manual jobs and so give patients chance to acquire a new start after his release .All these activities will help patients to kill boring long time and to use their energy in useful way.
3)General recommendation
• Increasing number of the staff and improving the level of medical service will help to improve ways of risk assessment and management. Improving the staff not only by increasing their number but also by improving their level, by establishing enough and effective training programs which raise their efficacy. Also to motivate any person, you should give him safe environment, put fair rules for reward and punishment and give him suitable financial level.
• Safety from dangerous environment containining uncontrolled dangerous patients is very important; this can be plentiful through increasing the number of trainable security teams and through presence of advanced tools as alarms and air doors which for example used in Finland Ian forensic psychiatric units.
• Release of patients from forensic patients should depend on improvement of their psychiatric disorders and level of risk not on the nature of crimes and the decision should be medical not legal, as these patients are not responsible and admitted in hospital till improvement.
• If nature of the crime is important from the legal point of view and should be put in mind before releasing the patients, suitable suggestion may be that improved patients can spend period which equal from 40% to 60% of the period spent by responsible person in the prison who committed similar crime .If we added risk assessment before release, it could be a good suggestion.
• The protocol which used after release must be followed. This protocol will obligate patients to take their treatment and so decrease incidence of relapse. This protocol means that any patient after release from forensic psychiatric unit should be followed up in the nearest psychiatric hospital monthly to be evaluated and to receive his treatment, and if any time he did not respect these rules, he should be obligate by force of the law. This may be for some as non ethical, but protecting the society and protecting the patient from hurting any one realizes more than this.
• The psychiatric law should protect the patients and the staff dealing with these patients .Listening to both before putting the law may help to resolve this dilemma.
