Abstract

In this retrospective study, the files of 535 patients with DTC who presented to ACOD from Jan. 2003 to Dec. 2012 were reviewed to study clinicopathological features and to evaluate treatment results.
The files of all patients included in this retrospective study were reviewed and data was collected including the following: complete history, clinical examination, histopathological data, types of surgery, radioiodine ablation, thyroid hormone suppression therapy, radiotherapy received, chemotherapy received and treatment results as regards disease free survival, progression free survival and overall survival.
In the current study, 82.8% were females and the median age of patients was 42 years.
Thyroid swelling was the most common initial presentation in DTC patients. Only 5% of patients were presented with distant metastasis.
Total thyroidectomy was the most common type of surgery performed in our patients, 85% of patients had papillary carcinoma, 14.1% had follicular carcinoma and 0.9% had Hurthle cell carcinoma.
Total thyroidectomy is the surgical procedure of choice for patients with DTC and is associated with better DFS than suboptimal surgery.
In this study, all patients received ablative doses of radioiodine and thyroid hormone suppression therapy with 12.3% of patients developed relapse (30 patients had distant metastasis, 28 patients had local recurrence, while 3 patients developed anaplastic transformation).
Lungs and bone were the most common site of distant metastasis in our patients.
In this study, there is no significant difference in DFS between patients received repeated low doses of RAI and those received high dose of RAI. Higher cumulative doses of RAI are associated with better DFS.
The 5 year DFS and OS was 89.2% and 100% respectively. There were factors affecting the DFS and OS including patients’ age, tumor size, soft tissue invasion, distant metastases and histopathological type of the tumor.
Treatment options for recurrent or metastatic disease, including: surgery, RAI, thyroid hormone suppression therapy, external beam radiotherapy and chemotherapy, are tailored according to the disease presentation.
