Abstract

Half of the maternal deaths occur within 24 hours of childbirth with postpartum hemorrhage (PPH) being the leading direct cause of maternal death in most developing countries. Many of the cases of PPH result from problems during and immediately after the third stage of labor(WHO, 2012). Waiting for a patient to meet the postpartum hemorrhage criteria, particularly in resource-poor settings or with sudden hemorrhage, may delay appropriate intervention of medical and nursing staff. Postpartum hemorrhage is a potentially life-threatening complication of both vaginal and cesarean delivery (Maame, 2010). To improve on the quality of care provided, it is of vital importance to evaluate the care.Intrapartum and postpartum periods are delicate periods in the life of every woman. It is thus important that optimum care be given to the woman during this period without which complications may arise (Philips. C., 2010). The researcher found that Nurses will know the quality of care they offer related to intrapartumand postpartum period to avoid or prevent post-partum hemorrhage as one of most common complication until it is being assessed, evaluated, reassessed and then reevaluated. This study on the effect of nursing intervention program on reduction of postpartum hemorrhage, therefore, will provide a means of evaluating the care and will provide a spring board for its improvement. The study was conducted with the aim to study the effect of nursing intervention program on reducing postpartum hemorrhage as a cause of maternal mortality and to improve nurses` knowledge and related to normal labor and post-partum hemorrhage management. Setting The study was conductedin the following sitting; Ashmon General Hospital and Teaching Hospitalin Shebein EL Kom at Menoufiya Governorate. Data collection tools: The tools of the study were designed by the investigator and were devoted to collecting data regarding to the nurses’ knowledge and skills during labour and postpartum periods and included the following: 1- Interview questionnaire of the nurse including: name, age, level of education, years of experience and knowledge related to postpartum haemorrhage,………etc. 2- Pre- test observational tool including three parts: a- Prenatal observational chick list . b- Natal observational chick list. c- Postnatal observational chick list. 3- Post- test observational tool including also three parts: a- Prenatal observational chick list . b- Natal observational chick list. c- Postnatal observational chick list. The tools were revised by a jury of qualified experts then validity and reliability were ascertained. Comments were incorporated into the tools. The main study findings are as follow - The majority of nurses of the study group was ranged between 28 – 20 years old of Middle\secondary nursing and years of experience that ranged between 11 – 3 years with poor knowledge and bad practice of the most skills. - There was highly statistical significant difference between pre and post intervention of the nurses` knowledge regarding normal labor and post-partum hemorrhage. - There was highly statistical significant difference(p<0.01) and statistically significant difference(p<0.05) between pre and post intervention regarding the majority of skills performance improvement during first, second , third stage of labor and first two hours after labor that mainly aimed to prevent post-partum hemorrhage. - There was statistically significant difference (p<0.05) between post and follow up intervention regarding the acquired knowledge related to normal labor and post-partum hemorrhage. - There was highly statistical significant differences and statistically significant difference between post and follow up intervention of the nurse`s skills and performance in first stage of labor regarding the majority of skills performance improvement during first, second , third stage of labor and first two hours after labor that mainly aimed to prevent post-partum hemorrhage. - Whether there was marked deterioration in nurses’ performance level if it is compared with the differences between pre and post intervention of the nurse`s skills and performance.
