Abstract

Pediatric asthma is considered the most common chronic pediatric illness nowadays, and it is rapidly increasing in prevalence. This evident rise of pediatric asthma prevalence necessitates continuous evaluation of the medical services. Aim of the work: This study aimed to determine the prevalence of bronchial asthma among primary school children in El-Menoufiya governorate (El-Bagour Center). Subjects: Children attending eight (8) primary school (5662 children) were surveyed constituting the target population. All asthmatics detected from the survey (155 asthmatics) were subjected for clinical history, physical examination, basic laboratory data and chest-x-ray. The results: of the present study showed: - The overall prevalence rate of childhood bronchial asthma in primary schools was 5.86 %. - The prevalence rate is higher in urban schools than rural schools as it is 6.58% and 4.86 % respectively. - In our study there is no sex predominance as male to female ratio 1/1.03 respectively The most common complaint of pediatric asthmatic patients was cough (96.1%), wheezes (91.6%), dyspnea (92.3%) and chest tightness (85.2%). - Allergic association were present in 82.6 % of asthmatic children as 40.6 % had allergic rhinitis, 26.5% had allergic dermatitis and 4.5 % had allergic conjunctivitis. - Exacerbation was induced among (59.4%) after viral respiratory infection, in (20.6%) on exposure to allergens, in (23,9%) on intake of special type of food, in (29%) after exercise and in (47.1%) on exposure to environmental factors. - In our study the level of education of parents and socioeconomic level had important role in the prevalence of asthma as ignorance and low socioeconomic level were associated with high prevalence of asthma 28.4% and 49.1% respectively but high education and high socioeconomic level were associated with lower prevalence of asthma 20.4% and 17.1% respectively . - In our study (30.3%) patients had family history of similar condition, (28.4 %) had bad housing condition, (70.3%) had exposure to smoking, (29.7) % had positive consanguinity and (24.5%) were in contact to animals. - The disease awakened (41.9%) of patients at night, (52.9%) of patients had missed school days, (16.1%) had limitation of activities and (18.1%) had visits to emergency room. - In our study (54.8%) of patients had mild intermittent asthma, (30.3%) had mild persistent asthma, (8.4%) had moderate persistent asthma and (6.5 %) had sever persistent asthma. - CBC in our study shows anemia in 33.5 % of cases, 44.5 % had eosinophilia and 9 % showed mild leucocytosis. Also parasitic infestation were present in 38.1% of asthmatic children’s In this work (57.4%) were on β2 agonist, (34.8%) on theophylline, (8.4 %) of patients were on chronic oral corticosteroid treatment, (23.9%) were on corticosteroid inhalers, (1.3%) on leuckotriene modifiers and (1.3%) were on sodium cromoglycate.
