Abstract

This study is concerned with comparison between conservative surgical treatment and Ray’s amputation in patients with diabetic foot osteomyelitis in Suez Canal University hospital.
The study was carried out in Suez Canal university hospital in Ismailia, the study patients included patients with diabetic foot osteomyelitis presented to surgery outpatient clinic.
The study population consisted of 108 patients with diabetic foot osteomyelitis divided into two groups, group A which resembled conservative surgical treatment while group B resembled Ray’s amputation, at any age in both gender.
The comparison between both techniques will be regarding many factors as hospital stay, healing time needed for wound closure, duration needed for return to work, patient’s satisfaction regarding public image and its psychological impact, and also assess the effect of different factors over wound healing as body mass index, smoking, type of diabetes and the duration of diabetes.
In our study, group A, 31 patients (57.4%) were discharged after one day from the hospital in comparison with group B that 8 patients (14.8%) were discharged in the same duration, that’s support group A technique as early discharge from hospital.
In our series, we found that 13 patients (24.1%) of group A returned to their daily activities or their work through a period less than 4 weeks in comparison to 3 patients (5.5%) of group B returned in the same duration, that’s made also the conservative surgical technique of group A is superior than the amputation technique of group B.
In the presented study we found there are closed relationship between the healing duration as wound closure and smoking, body mass index, type of diabetes and the duration of diabetes.
Conclusion
Conservative surgical treatment is superior to amputation of diabetic foot osteomyelitis regarding hospital stay, healing time needed for wound closure, duration needed for return to work, patient’s satisfaction regarding public image and its psychological impact, and there are many other factors affect the healing duration as smoking, body mass index, type of diabetes and the duration of diabetes.
