Abstract

Serious bacterial infection can be difficult to distinguish from viral infection; the consequences of a delayed or missed diagnosis can be serious and occasionally fatal
Our study is a prospective diagnostic accuracy study aims to improve management of serious bacterial infections in children aged less than 5 years old. The study included 416 children presenting with febrile illnesses who attended the emergency department of Ismailia Fever Hospital from November 2011 to April 2012. We exclude 16 children and the total number of included children was 400[194(48.5%) females and 206(51.5%) were males].
All patients were subjected to the following.
- Initial clinical assessment by the physicians and any test could be ordered during this assessment on the basis of clinician judgment.
-A questionnaire consists of personal data, symptoms, and signs that were filled by the physician after the initial clinical assessment and before any test result were obtained. The physicians estimated the probability of the potential diagnosis which cover serious bacterial infection, clinically apparent infection and viral illnesses.
- All patients were diagnosed by using standard radiological (chest x-ray) and microbiological methods (urine analysis and culture, CBC, CSF analysis) as having serious bacterial infection (pneumonia, urinary tract infection, meningitis), clinically apparent infection, or no bacterial infection.
