Abstract The promotion of therapeutic adherence is considered as an integral component of pharmaceutical care practice and patient healthcare. It has been shown that despite effective methods of’treatment, 50% of diabetic patients fail to achieve satisfactory glycemic control, which leads to accelerated development of complications and increased mortality. Aim of work and objectives Toimprove the quality of diabetic care in Fanara center affiliated to Suez Canal University, improve adherence of diabetic patients towards different therapeutic regimens and assess patterns and obstacles of oral antidiabetic medications adherence. Subjects and Methods A total of 376 Type 2 diabetic patients who fulfilled the inclusion criteria were recruited in the current study. Adherence to the treatment was evaluated in Fanara center. The medication adherence had been assessed during a personal interview with each patient using a multiple-choice questionnaire. Also using the Measure Treatment Adherence scale to assess the factors affecting the adherence and the adherence scale among the patients. Results Our study results showed that, 193 patients (52.39%) had poor knowledge about the disease and its complications. As regards to the factors affecting the adherence,255 patients (67.82%) had weak believes and motivations, on the same time 230 patients (61.17%) had poor communications with health care providers,272 patients (72.34%) were not regularly monitoring blood glucose level. Concerning the relation between variables; there was a highly statistical significant effect among studied population between different gender and direct and indirect cost income,(p=.0005) also there were relationship between patient believes and motivation, patient health care providers relationship and family support(p=.04),(p=.02) respectively, while there were no statistical significantrelations with other factors.As regards to assess the adherence rate, we used the MT A scale which was consisting of six questions, 178 patients (47.09%) had fair adherence, 100 patients (26.59%)had good adherence rate,while 98 patients (26.32%) had non adherence rate. ~ Conclusions Evaluation of adherence is vital for patients with diabetes in order to manage them also the adher~l1ce rates to medication, dietary/exercise and appointment were observed to be suboptimal. These causes are call for improvement of the adherence through continuing patient education about diabetes, improvement of patients’ economical levels as well as a reduction in the cost of medication.
