Abstract

Neonatal care has made tremendous improvements in developing countries. However, there are number of challenges to be met as neonatal mortality remains unacceptably high. In contrast to this, neonatal care in developed nations has moved ahead of a pre-occupation of reducing the neonatal mortality only. The main reasons for this gap are poor infrastructure, resource limitations and lack of systems developed by neonatal units in the developed nations.
The NICU of Ismailia General Hospital provides level III neonatal care was constructed in year 2005 but a main start of the NICU was the innovation done to the NICU in year 2007. It is now equipped by 15 incubators, 2 mechanical ventilators with 3 CPAPs and 10 phototherapies.
This study describes the epidemiological aspects of all neonates admitted along one year (2011) in NICU of Ismailia General Hospital.
Medical records were examined all neonates admitted in 2011 through the admission data, NICU stay and discharging outcome. The data were collected via a questionnaire till analysis was achieved. It included patient’s admission data (e.g. demographic data, mode of delivery, gestational age), NICU stay data (e.g. average duration of stay, requirements for assisted ventilation) and discharge data (e.g. clinical outcome). However, it is worthy to say that this study was restricted by the limited data resources.
The total number of cases admitted to NICU of Ismailia General Hospital along year 2011 was 397 neonates. Among cases of admission, 244 were males while 153 were females, 19.1% were preterms while 80.9% were full terms. The overall mortality rate was 19.1%. Mode of delivery and gender didn’t affect the clinical out come. On the contrary, gestational age, mechanical ventilation and certain diagnoses affected the clinical outcome significantly.
The commonest causes of admission were respiratory distress (53.9 %), jaundice (53.4 %), and prematurity (25.4%). While the highest cause of mortality was prematurity (19.1%) and respiratory distress (19.1%).
The most commonly recovered microorganism from blood cultures was staph. aureus (48.3%).The with mean hospital stay of (8.9±7.9days). The highest mean of hospital stay was recorded for sepsis (20.2±9.7days) and NEC (17.1±9.4 days). (21.4%) of the mechanically ventilated were weaned with success. Yet mechanically ventilated neonates represent 57.9% from total deaths.
Other prospective and retrospective studies are highly recommended and to make these studies much easier and of greater benefit and accuracy, we recommend constructing a proper filing system for all neonates admitted to NICU of Ismailia General Hospital by creating a computerized data entry program.
