Abstract Supraorbital keyhole approach is a minimized variation of the frontal part of classic pterional approach through which the anterior cranial fossa and perisellar structures are free for dissection and the tip of the temporal lobe do not obstruct field The keyhole concept is based on the fact of the cone of vision. The intracranial optical field widens with increasing distance from the craniotomy. Supraorbital keyhole approach is not a slandered one size approach that fits all skull base and sellar lesion but it is a highly selective approach that is tailored according to individual anatomical and pathological criteria of each lesion Technical advantages of neuroendoscopes allowed for their use during ordinary microsurgical procedure. They provide advantages of more illumination, wider field of view, sharp image quality and ability to inspect hidden corners.CT and MRI of the brain were performed in every patient and meticulously studied to ensure suitability of this approach to access the lesion and selection of the surgical corridor. |
