Abstract

Parathyroid hormone is a middle sized molecule with a molecular
weight of 9500 Da. Hyperparathyroidism is a common finding in patients
with renal insufficiency. Calcitriol deficiency and phosphate retention
together with hypocalcaemia are main factors involved in pathogenesis of
secondary hyperparathyroidism.
Excess parathyroid hormone causes a rise in intracellular calcium,
disturbing the functions of virtually every organ system, including bone
mineralization, pancreatic response to metabolic and hormonal changes,
erythropoiesis, the immune system, the heart, the liver, or the brain.
The aim of the study is to assess the effect of high-flux versus lowflux
dialysis membranes on parathyroid hormone in chronic hemodialysis
patients.
The current study comprised 40 adult hemodialysis patients. These
patients were enrolled from Hemodialysis Unit,Benha Teatching Hospital
after fulfilling of our inclusion criteria. These patients were subjected to
full history taking and clinical examination.
Laboratory evaluation was undertaken including:
- Routine investigations including CBC, serum calcium level,
phosphorus level, Serum creatinin , blood urea nitrogen, and albumin
were measured at the beginning of and 1 month after the use of high flux
dialyzer.

- Quantitation of intact parathyroid hormone was measured by Enzyme
Linked Immunosorbant assay (ELISA) before hemodialysis session and
30 minutes after HD session. Intact PTH was remeasured before and 30
minutes after the hemodialysis session after 1 month of use of high flux
dialyzer.
In our study we found postdialysis highly significant decline of
intact PTH after the use of high flux membranes,but not after the use of
low flux ones. Also predialysis intact PTH level after the use of high flux
membranes showed a highly significant decline compared to the
predialysis intact PTH with low flux membranes at the start of the study.
