Abstract

Diabetes is a group of metabolic diseases characterized by hyperglycemia resulting from defect in insulin secretion , insulin action or both the chronic hyperglycemia of diabetes is associated with long term damage,dysfunction and failure of different organs especially the eye , kidney, nerves,heart and blood vessels.
Thrombomodulin (TM), cd 141 or BDCA-3 is an integral membrane protein expressed on the surface of endothelial cells and serves as a cofactor for thrombin. It reduces blood coagulation by converting thrombin to an anticoagulant enzyme from a procoagulant enzyme. Thrombomodulin is also expressed on human mesothelial cell, monocyte and a dendritic cell subset.
The aim of the present study is to assess the serum thrombomodulin (TM) level in type 1 diabetic patients according to their glycemic control
• This study was conducted on 60 individuals who devided into two groups:
• Group A: 40 individuals 22 of them were controlled the remaining 18 were uncontrolled type 1 diabetic patients.
• Group B: 20 apparently healthy individuals.
• All individuals included in this study will be subjected to the following:
1. Full history taking: focusing on family history of type 1 diabetes mellitus, hypertension and smoking.
2. Thorough clinical examination
3. Laboratory investigations that include:
a. Fasting blood glucose level.
b. Post prandial blood glucose level.
c. d. Routine lab chemistry (liver, kidney functions).
4. Serum thrombomdulin level assay by enzyme linked immunosorbent assay (ELISA) one-step sandwich enzyme immunoassay.
5. Statistical analysis of data.
Our data shows clearly that type 1 diabetic patients in (group A) were statistically significantly higher in TM levels than control group thrombomodulin level increases by increasing FBS, PP BS, urea and creat level.
Soluble thrombomodulin level found to be correlated with endothelial cell injury in patients on hemodialysis atherosclerotic and diabetic patients.
