Abstract

C
omplications of cardiac implantable electric devices (CIED) represent a major problem in clinical practice due to increased rate of implantation of CIED, especially cardiac resynchronization therapy device (CRT) and implantable cardioverter defibrillator (ICD).
Several classifications for CIED complications exist, they are classified according to implantation time into acute or chronic, according to the site of complication into lead or pocket complications, and according to etiology into implantation or system failures. Implantation-related complications are the most common.
The current study was done to show the incidence of complications of cardiac implantable electric device in patients presenting to the CIED clinic at Ain Shams University Hospital.
It included 449 patients with CIED who presented to Ain Shams University Hospital in the period from March 2013 to August 2013. Each patient was subjected to thorough history taking to detect occurrence of any complications, examination and programming of the device.
Incidence of CIED complications in the patients was 11.13% (4.67% for pocket-related complications and 6.46% for lead-related complications).
CDRI is the most common pocket-related complication, and may necessitate re-intervention in the form of battery exchange and/or lead extraction.
The incidence of lead-related complications was significantly higher in patients with history of smoking.
There is a statistically higher incidence of complications with dual chamber than single chamber device due to increased incidence of right atrial (RA) lead dislodgement which is considered the most common lead-related complication.
It is recommended that attention to strict infection control measures when dealing with the device is essential, as well as proper positioning of RA lead in dual chamber device.
