Abstract

This is retrospective study carried out in pediatric Hospital, Faculty of Medicine, Ain Shams University to evaluate the economic burden of acute diarrheal diseases, started in May 2010 and completed one year later in April 2011.
Data were collected by analyzing patients’ files and hospital records through Excel (Microsoft) spreadsheets to determine full clinical history, physical examination, therapeutic medication, laboratory investigations, death events, I.C.U cases and economic burden of acute diarrheal diseases.
Hospitalized cases of acute diarrhea represented 8.4% (n=1069) among total number of admission cases over the same period, diarrheal diseases were more common in infants less than 8 months , 59.1% of studied cases were males, 53.2% of diarrheal cases occurred during warm season and 41% of studied cases were complicated gastroenteritis.
Breastfeeding was a protective factor against infection associated diarrhea, 70.6% of the studied cases were non breastfed.
Vomiting was the most common associated symptom in 77% of studied cases followed by fever. Dehydration was the most clinical sign, 74.2% of cases had mild-moderate dehydration and 25.8% had severe dehydration.
Median length of hospital stay was 3.6 days. 91.2% of cases received I.V fluids, 89.5% of case received antibiotics and only 12.9% of studied cases received O.R.S.
Mean total cost per each hospitalized case of acute diarrhea was 710 LE (Egyptian pounds), 84% was medical cost while 16%was non medical cost. Medical cost included costs of facility, professional, medication and diagnostic tests.
Higher IQR of total cost and hospital stay among cases below 8 months of age compared to older cases and the difference was significant statistically.
Higher median cost and hospital stay among complicated cases compared to simple cases of GE and the difference was highly significant statistically.
Higher median total cost and hospital stay among cases admitted to ICU due to severe dehydration and the difference was significant statistically.
proportionate mortality rate of diarrhea represented 9.7% of all deaths of pediatric hospital Ain Shams University and Years Life Lost due to premature mortality (YLL) were 936 years.
According to our results and results of previous studies, the introduction of a rotavirus vaccine in Egypt’s Expanded program on Immunization should decrease the high economic burden of acute gastroenteritis.
