Abstract
Real-time sonographic guidance in conjunction with an automated 16- 18 gauge core biopsy system is a safe and accurate method in hand to perform percutaneous renal biopsy and can be safely performed as an outpatient procedure reduce the complication rate while allowing the retrieval of sufficient tissue for histological diagnosis.
There were many indications for renal biopsy include diffuse renal disease and focal renal diseases as (mass ,cysts) & many contraindications either relative as uncontrolled hypertension ,hypotension & etc, or Absolute as bleeding diathesis ,anatomic abnormalities & etc .
There are many complications either (minor gross hematuria, Silent hematoma) Major (hematoma requiring transfusion and/or embolization)catastrophic (Loss of functional mass, Death).
This study included 30 patients with various indications of renal biopsy 18 males & 12 females with age ranging between 3 and 63 years. All patients underwent detailed history taking, clinical examination and many labrotary investigations were done, and then ultrasound examination of the kidney and US guided renal biopsy was performed.
In this study , manifestation of renal affection, manifestations of acute kidney injury were seen in 4 cases , Lupus nephritis were seen in 2 cases manifestations of renal impairment were seen in 17 cases, delayed milestones were seen in 2 cases , bilateral pleural effusion were seen in 1 case generalized anasarca were seen in 2 cases with percent about , hypertension were seen in 3 cases , 1 case with convulsion , back pain was seen in 3cases and cases of the above had combined presentation.
In this study , the investigations done to patients with renal biopsy including PT ranged from 75 to100 %, Hb ranged from 5.5 to 14, HCV 3 cases with positive HCV and 27 cases of negative HCV , urine analysis proteinuria 4 cases were ++ and 26 cases were +++,ANA 5 cases were positive and 25 cases were negative.
In this study the ultrasound finding in 24 cases with diffuse renal pathology was 10 cases of normal kidneys findings , 1 cases with bilateral enlarged kidneys , 10 cases with bilateral grade II nephropathy ,3 cases with bilateral grade III nephropathy and in 6 cases with focal renal pathology was 1 case of bilateral renal mass, 5 cases of left renal mass .
In this study, we had 6 cases of focal renal pathology, 5 cases were malignant and 1 case was benign. And 5 cases were of solid nature and 1 case of mixed solid and cyst components.
In our study ultrasound guided renal biopsy done in 30 cases we had 24 cases of diffuse renal pathology and 6 cases of focal pathology , the biopsy results were , 4 cases with focal segmental GN , 6 cases with membrano-proliferative GN , 4 cases with lupus nephritis , 2 cases with mesangial prolfierative GN , 4 cases with chronic scelrosing glomerulosclerosis , 2 cases with multiple myeloma, 2 cases renal amylodosis .3 cases with renal cell carcinoma, 2 cases with lymphoma and 1 case with metanephric adenoma .
In this study, we had free handed biopsy technique in 15 cases with 6 complicated cases and 15 cases with automated gun biopsy with 4 complicated cases.
In this study the complication of the renal biopsy was seen in 10 cases of 30 cases , 10 cases shows hematuria, 20 cases show no complications . the size of the needle used are 14G,16G and 18 G and the rate of complications was more using 14 G and less using 16 G& 18 G needles and Number of the needle passage and its relation to complication, complications were more in 4 time passage and less in 2 and 2 time.
