Abstract

This prospective randomized study included 40 patients with CSM
divided into 2 groups; 20 patients had ACDF using PEEK cages without anterior
platting (Group 1) and 20 patients had ACCF using TMC augmented by anterior
locked plate (Group 2).
All patients were scored for: myelopathy using JOA score, neck pain and
brachialgia using VAS preoperatively, postoperatively and followed up at 6w,
12w, 24w and at 1y both clinically and radiologically for extent of fusion,
restoration of cervical lordosis (using modified Ishihara cervical curvature index)
and positioning of the cage.
At 1 year follow up, myelopathy improved in 95% of each group. Pain
scores improved significantly with no significant difference between both groups.
Complete fusion was achieved in 95% of each group at 1 year without significant
differences. Modified Ishihara index improved significantly in all kyphotic
patients.
