Abstract

from July1993 to November 2010, 67 patients with traumatic brachial plexus palsy in children, with exclusion of birth injuries, were operated for brachial plexus exploration and reconstruction. . Only 52 children were available for complete follow up. out of which 43 (82.6%) were males and nine(17.4%) were females. The mean age at surgery was 7.9 years (2- 16years). Patients were followed up for 24 months - 152 months (mean 52.83 months). The right side was affected in 24 (46.2%) cases and left side in 28 (54.8%) cases. It was found the motor cycle accident was the most common causes of injuries. Ipsilateral diaphragm affection was found in 9.6% ’of cases, while Homer’s syndrome was noted in 11.5% of cases. EMG and NCV were used in all cases ,where it was done at least after 3 week from the trauma to assess the nerve conduction and level of injuries of the affected part. ssep results were more sensitive to nerve rupture than avulsion. roentgenograms of chest, clavicle, shoulder, and elbow were obtained to diagnose associated fractures and elevated copula of diaphragm, indicating phrenic nerve injury. CT myelography was done for 11 cases, CT myelography sensitivity was 52.3% and its specificity was 52.9% and it was more sensitive, 75%, and specific, 71.4%, to C8 lesions. Images were examined for the presence of vertebral fracture, spinal canal alignment, intraspinal canal haematoma, meningocele and nerve root visualization.
