Abstract

Several bariatric procedures are available for patients with morbid obesity. Numerous researches discussed the efficacy of the various bariatric procedures, particularly in short term. Data concerning the long term efficacy of these procedures is heterogonous.
This study aims at a retrospective analysis of the long term efficacy of the vertical banded gastroplasty (VBG), gastric banding, and gastric by-pass (RYGBP).
The study was conducted in collaboration between the HBP surgery unit, Alexandria main university hospital, faculty of medicine, Alexandria University and the department of minimal invasive surgery, El Molinetta Hospital, University of Turin, Italy. Data concerning the long term weight changes and complications of the VBG, gastric banding, and RYGBP were retrieved from a large bariatric data base.
A total of 3515 cases have completed three years post intervention and were included in the study, of them 610 were males and 2906 were females. The mean pre-operative weight was 120.3 kg (range= 63-241kg), and the mean pre-operative BMI was 44.5 kg/m2 (range= 32-118). The mean excess weight percent for males was 104.8 % and 107.3 for females. The mean BMI for males was 45.7 kg/m2, while for females was 44.2 kg/m2. The mean age was 38 years (SD 10.4)
1668 patients underwent VBG, of them. 1602 patients underwent gastric banding. In 245 patients, RYGBP was performed. Of all patients, 1128 patients underwent an open procedure, while 2387 underwent a laparoscopic procedure.
Among patients who underwent VBG 282 were males and 1381 females, while 55 males and 190 females underwent by-pass. 286 male patients and 1334 female patients underwent gastric banding.
At the scheduled three years follow up visit, only 2214 patients` visit records (62.98%) could be retrieved. The mean BMI at three years was 32.4 kg/m2, and the mean excess weight loss percent was 26.3% for males versus 48.9% for females.
1500 records were retrieved for 4 years follow up, where the mean BMI was 33 kg/m2 and the mean excess weight loss percent was 23% for males and 49.5% for females. The mean BMI was 53.8 kg/m2 for 1671 records that represent 5 years duration of follow up. For them the mean excess weight loss percent was 30.7% for males and 49.7% for females.
