Abstract

Objective: To investigate the efficacy of gonadotropin releasing hormone(GnRH) antagonist in poor responders undergoing in vitro fertilization/ intracytoplasmatic sperm injection(IVF/ICSI).
Study design one hundered and twenty patients of patients assuming to be poor responders were prospectively randomized into two groups. Fifty-seven patients were stimulated with GnRH antagonist multidose protocol and fifty sex patients received a standard long agonist protocol. Ovarian response was evaluated by transvaginal ultrasound. Cycle characteristics and treatment outcomes were statistically compared between groups.
Results: High cancellation rate in agonist which is more than in antagonist arm but no significant differences between the two groups. There was reduced duration of stimulation and consumption of gonadotrophins in the antagonist group when compared to the agonist group which reach significant values in consumption of gonadotrophins. The number of oocytes retrieved, and the number of embryos transferred were similar for both groups. No statistically significant differance in clinical pregnancies were achieved
Conclusion flexile multiple dose antagonist protocol in poor responders may reduce consumption of gonadotrophins, however no significant differences in pregnancy outcome.
