Abstract

Type 1 diabetes represents a considerable burden to economy and society and remains a major challenge. The treatment of choice for type 1 diabetes consists of exogenous insulin therapy with tailored diet and physical exercise. No formulation of exogenous insulin available to date has yet been able to mimic the physiological nictemeral rhythms of this hormone. So, the replacement of β-cells through whole pancreas and pancreatic islets transplantation are the only concrete alternatives for re-establishing the endogenous insulin secretion in type 1 diabetic patient.
Islets transplantation is associated with high rate of early graft failure; it is probable that the destruction of the islets micro-environment by digestive enzymes (collagenase) that occurs during islets isolation would impair beta-cell function and survival.
The purpose of this study is to focus on the effects of the main extracellular matrix components ”collagen I and IV , fibronectin and laminin ” on islet B-cell adhesion , survival and function and the value of extracellular matrix protein coated scaffolds as a technique to enable the creation of well-defined micro-environment which will promote graft efficacy at extrahepatic implantation sites .
Conclusion: Diabetes mellitus is big health problem and its complications affect all organ systems all over the body. Islet transplantation is the ideal treatment for T1DM. Islet transplantation remains faces many difficulties, the major one is islet cells loss in tissue culture. Use of ECM-coated scaffold improves islet cell survival in tissue culture, which is promising for increase in success rate of pancreatic islet transplantation.
