Abstract

PW-TDI has been used to evaluate velocities and time intervals of the contraction and relaxation of both ventricles in different clinical situations (Moreno R et al., 1999&2001) However, there are very few studies that have provided quantitative data on atrial function using PW-DTI (Iglesias I., 2002).
The aim of this study is to analyze the profile of left atrial wall velocity by pulsed wave tissue Doppler parameters in diabetic patients, and its relationship with the transmitral velocities and the left atrial pump function parameters obtained by conventional echocardiography for early detection of diabetic cardiomyopathy.
In this study, 30 type II diabetic persons presented in Ain Shams University Hospital in the period from March 2009 to May 2010 were divided into two groups: group (1): persons in whom the change in peak Simpson’s EF from the resting Simpson’s EF was > 5%, group (2): persons in whom the change in peak Simpson’s EF from the resting Simpson’s EF was < 5%.They were subjected to the following:
1. History evaluation, clinical examination, resting ECG.
2. Transthoracic echocardiographic examination: Measurement of A wave, E/A ratio, left atrial diameters &volumes, left atrial ejection force, left atrial ejection fraction, left ventricular ejection fraction by M-mode & modified Simpsons’ techniques.
3. Dobutamine stress echocardiography.
4. Pulsed wave tissue Doppler imaging of the left atrial lateral wall.
Subjects with the following conditions were excluded from the study: Age above 60 years, hypertension, IHD, LV systolic dysfunction.
In comparison between group (1) and group (2) the left atrial ejection fraction and left atrial stroke volume were found to be highly significantly lower in group (2) compared to group (1).The pulsed tissue Doppler A2/A3 ratio at the basal, mid and distal segments of the left atrial lateral wall was significantly lower in group (2) compared to group (1).
from the present study we found that Pulsed TDI of the left atrium & left atrial ejection fraction by conventional echocardiography at rest could identify subclinical diabetic cardiomyopathy in normotensive type II DM.
