Abstract

Distal embolization is a well-recognized complication of vein graft intervention. Embolization compromises the distal microcirculation, and manifests as no-reflow, and CK-elevation. Distal embolization can cause catastrophic consequences, especially in older vein grafts with soft friable plaques. As early as 1990, Theron, the pioneer of cerebral protection developed and first advocated the use of a distal protection device. The lessons learned from distal protection of cerebral circulation has resulted in launching of several small scale studies looking at different protection devices for vein graft interventions.
The aim of this randomized study is to evaluate the safety and efficacy of the PercuSurge GuardWire® system during percutaneous intervention of saphenous vein bypass grafts. The PercuSurge GuardWire® treatment will be compared to the current standard care of SVG interventions, including PTCA &/or stenting. The study is being conducted concurrently while the SAFER trial results are awaited. Results of this study will shed more light on distal protection.
