Abstract

Summary
Surgery is the main stay of treatment for many of thyroid lesions. Nursing has always played a central role in the management of patients undergoing thyriodectomy and its complications. This study was conducted at surgical units of Assiut University Hospital.
The aim of the study was to assess nursing care for patients undergoing thyroidectomy and post operative complications that face those patients.
To accomplish this aim; data were collected from all available nurses who were currently assigned for giving care of fourty (40) adult patients who underwent thyroidectomy in the period from January to September (2008).
The following tools were utilized for data collection; two for nurses; questionnaires sheet and observation checklist. One for patients; Patient assessment sheet.
Data was collected & analyzed by computer programmed spss (ver.13). Using necessary statistical manipulations as; number, percentage, mean & chi-square test to determine significance for non- parametric variables. T. test used to determine significance for numerical variables.
Main finding of the present study were:
According to nurses:
 Mean age of nurses was (29.53  12.41). Females were the majority (72.22 %), nursing diploma was the highest proportion 94.4 %. None of them got the opportunity of attending any previous training program about nursing care for patients undergoing thyroidectomy.
 Most of nurses (94.3%) had unsatisfactory level of knowledge about nursing care for patients undergoing thyroidectomy with a mean value (81.49  10.45), while only 5.7 % of nurses had satisfactory level of knowledge with a mean value (143.25  14.31).
 More than half of nurses 66.67 % had adequate practice. with a mean value (42.53  10.32) for the total score of their practice.
According to patients:
 More than half of the patients (67.5 % ) have an age ranged from 30- > 44 yrs. and most of the them (87.5) were female.
 As regard histopathological diagnosis of the patients included in the study. It was found that, More than half of female patients (54.3 %)& less than half of male patients (40 %) had simple goiter, (17.1 %) of female patients had toxic goiter & also the same percent of female patients had thyrioditis. There was 11.4 % of female patients & 20 % of male patients had thyroid tumor, with no significant difference between gender & diagnosis ( P > 0.05).
 As regard surgical intervention; 51.4 % of female patients had undergone subtotal thyroidectomy & 40 % of male patients had undergone lobectomy & the same percent of male patients had undergone total thyroidectomy, while female patients who had undergone thyroid lobectomy & total thyroidectomy were (14.3 %, 22.9 %).
 As regard postoperative complications; some patients developed some complications in different times; as obstructed airway , Hemorrhage, Tetany, R.N.I, pneumonia and G.I.T complications with the following percentage (5 %, 12.5 % , 10 % , 12.5 %, 5 % , 25 %) during 24 hours post operative.
 After 24 hours post operative appeared some complications as hemorrhage, tetany, R.N.I.& pneumonia (2.5 %, 5 %, 12.5 %, 5 %)
 Tetany & R.N.I. continued to develop in some patients until time of suture removal (5-7 days postoperative) with the following percentage (15 %, 7.5 %).
 Ten days after suture removal; 20 % of patients developed wound infection.
 In time of follow up (after three months postoperative) there were 5 % of patients had Tetany & the same percent of patients had R.N.I.
 there was no significant relation between nurses’ knowledge & their years of experience.
 there was no significant relation between nurses’ practice and their knowledge.
 There was significant difference between nurses’ practice & their age groups. It was noticed that the highest percentage of adequate nursing practice was at age group 20-25 yrs. while the lowest percentage was at 39 ≥ 40 yrs. And lowest percent of inadequate nursing practice was at age group 20-25 yrs.
