Abstract

The aim of this work was to study the thyroid function in some cases of COPD with prolonged mechanical ventilation and to study the weaning pattern in relation to the thyroid function.
This study was conducted upon 20 COPD patients with prolonged mechanical ventilation, admitted to the respiratory intensive care units of Ain Shams University hospitals.
Admission history, clinical examination and laboratory investigations revealed:
• 12 patients (60%) had ESS, 4 patients (20%) had normal thyroid function and 3 patients (15%) had hypothyroidism, while only one patient had subclinical hyperthyroidism.
• L-thyroxin replacement therapy was given to only one patient of the 3 cases of hypothyroidism and he was successfully weaned from MV after 7 days of receiving L-thyroxin. The other 2 cases of hypothyroidism did not received L-thyroxin replacement therapy as they had died before the results of thyroid function were available.
• 12 patients (60%) had ESS with mortality rate of 50% among this group of patients.
• Patients with ESS (12 patients) had been divided into 2 groups. The first group was given L-thyroxin replacement therapy (100µg/day), while the second group did not. Both groups were observed for the outcome and thyroid function was repeated after one week. It was noticed that the mortality rate in the group receiving L-thyroxin was higher (66%) than that in the second group (33%). But, this was of no statistical significance due to the small size of the studied group.
Also; L-thyroxin did not normalize T3 levels. So, it was concluded that L-thyroxin replacement therapy did not improve the survival in Patients with ESS.
• As regards the outcome of patients in our study, it was found that 45% of patients were successfully weaned from MV, while 55% had died.
• There was no significant relation between FT3 and mortality rate, while there was a highly significant relation between APACHE score and the mortality rate.
• The BMI had a significant relationship with the outcome of patients.
