Abstract

Portal hypertension is a common and serious problem in children,
various portosystemic collateral channels develop as a result of portal
hypertension.
Oesophageal varices develop as a result of portal hypertension, and
bleeding oesophageal varices is a major complication. Endoscopic

sclerotherpay, and pharmacologic vasoactive drugs are used for proper
control of acute bleeding episodes; prevention of early rebleeding, and in
eradication of oesophageal varices.
The aim of the present study was to evaluate the efficacy of injection
sclerotherapy in control ofacute bleeding in comparison with the additional
use of octerotide for proper control of acute bleeding and prevention of
_......-- early rebleeding, also to evaluate the efficacy of sclerotherpay on variceal
eradication and prevention of recurrence.
The study comprised 40 children who were divided into 2 groups.
All children had oesophageal varices as detected by upper gastrointestinal
endoscope. The first group, 20 children subjected to endoscopic
sclerotherapy only while second group subjected to combined injection
;.
sclerotherapy and octerotide infusion for 3 days after sclerotherpay session
in a dose of 1 ug/kg/hour.
All cases of the 2 groups were subjected to the following
investigations:
• Complete blood count.
• Liver function test.
- ’.
• Prothrombin time.
Hepatitis markers including hepatitis B surface antigen and hepatitis C
antibodies.
Liver biopsy for selected cases.
Doppler ultrasound.
Abdominal ultrasound.
