Abstract

:In patients suspected of having brain tumors, magnetic resonance imaging (MRI) is considered to be the reference standard for preoperative diagnostic evaluation and for providing optimal information for clinical decision making.Although conventional MRI is a useful method ,it is not always possible to determine brain tumor grade The potential of non- invasive in vivo-proton magnetic resonance spectroscopy (MRS) for determining the type and grade of brain tumours has been shown in a number of studies.Cholin (Cho) is known as a radiological marker suggesting cell turnover. Brain tumors mostly demonstrate elevation in the ratio of Cho/Creatine(Cr).
The aim of this work is to study the role of conventional MRI & MR spectroscopy in the evaluation and grading of brain gliomas
:MRS proved to be more accurate in detection of brain gliomas as well as grading . Sensitivity, specificity, PPV, and NPV for determining a high-grade glioma with conventional MR imaging were 43%, 82%, 50%, and 78% respectively. Sensitivity, specificity, PPV, and NPV for determining a high-grade glioma with MRS were 86%, 94%, 86%, and 94% respectively. MRS provide valuable information related the biochemistry of tissues that plays a diagnostic and prognostic role in the evaluation of brain gliomas and some of the common brain tumours. It can differentiate between low and high grade gliomas as well as metastatic tumours. Also differentiate tumour recurrence from post radiation necrosis.
