Abstract In normal kidneys , we recorded a mean ADC value of 2.87+ 0.50 x10-3 mm2 /s at b value = 800 s / mm2 . Our data provide values which are similar to most of those data reported in literature to date . The test is feasible and reproducible that means that repeating the test at the same parameter on the same circumstances after time interval shows no statistically significant difference .Age and sex does not affect ADC value .The mean ADC value was much reduced in kidneys with diffuse parenchymal disease with statistically highly significant difference between mean ADC value of normal kidneys and mean ADC value in diffuse parenchymal disease .We could not differentiate different pathological types of diffuse parenchymal disease depending on ADC value of each type .The mean ADC value was much reduced in kidneys with focal parenchymal lesion with statistically highly significant difference between mean ADC value of normal kidneys and Mean ADC value in focal parenchymal lesions .We could not differentiate between benign and malignant focal lesions and we could not differentiate different pathological types of malignant focal renal lesions depending on ADC value of each type. In conclusion, on the basis of this preliminary experience, DW MRI of the kidney seems to be a feasible and reliable method to differentiate normal renal parenchyma from different renal parenchymal diseases , on the other hand , we could not differentiate between different pathological types of diffuse parenchymal diseases , normal parenchyma & focal parenchymal diseases and different pathological types of malignant focal lesions . This technique could be applied in the clinical area as a rapid addition to existing kidney MRI protocols since the acquisition time of the sequence is only 60-120 s and thus provide DW images of diagnostic quality as well as quantitative data regarding diffusivity. However, clinical experience with this method is still preliminary and further studies are required to validate the present results and to better elucidate the clinical application of this technique. |
