Abstract

Osteoarthritis is considered one of the most common chronic diseases and a major cause of disability which leads to decline in physical activity and may ultimately require joint replacement surgery. Knee joint is commonly affected with OA. Some studies suggested that OA is linked to atheromatous vascular diseases and that OA incidence and progression may be primarily because of the affection of the subchondral bone arteries with the cardiovascular risk factors called the metabolic syndrome. The present study was designed to assess the effect of the MS on the severity of knee OA. In this study, 60 female patients with a clinical diagnosis of primary knee OA were recruited from the Rheumatology outpatient clinic at Kasr Al-Eini Hospital over the period from 5/2007 to 11/2007. Their ages ranged from45-55 years (mean 51 .6± 3.3 years) and they were divided into two groups: Group (A): 30 patients with knee OA and MS. Group (B): 30 patients with knee OA and without MS. Both groups were matched as regards: age, disease duration, BMI and specific occupational risk factors for knee OA. All subjects fulfilled American College of Rheumatology (ACR) clinical criteria for knee OA. Patients were excluded if they had traumatic or inflammatory arthritis or major concurrent illness or receiving currently physiotherapy for knee pain. Conclusion: In summary this study confirms that presence of the metabolic syndrome in female patients with knee osteoarthritis (aged 45-55 years) is associated with more severe clinical signs, more progressive radiological damage, more severe grades of functional disability and more frequent affection with generalized osteoarthritis compared to osteoarthritic patients without metabolic syndrome. The control of these metabolic factors may hinder progression of knee osteoarthritis.
