Abstract

Hepatocellular carcinoma is an important disease worldwide, generally leading to death within 6-20
months.
As the prognosis is so good in early disease and so bad at later stages when diagnosis is usually made, it seems logic to screen for small HCC in all cirrhotic patients. ( Masahiko et al, 2006 ).
This study was carried out on 45 hepatic patients present mainly by ascites. Also, may be presented by
other complications of liver cell failure such as hepatic encephalopathy, spontonous bacterial peritonitis and
upper GIT bleeding. Those patients were selected from Ain Shams University inpatient and outpatient departments in the period from (6/2008) to (3/2009).
The patients were divided into 3 groups: Group A(cirrhotic patients with ascites), Group B (HCC patients
with ascites) and Group C (SBP patients with ascites).
The aim of this study is to detect the value of serum ascitic albumin gradient (SAAG) and ascitic fluid triglycerides in prediction of hepatocellular carcinoma.
And also comparing the SAAG and asitic fluid triglycerides between the three groups included in this study (cirrhotic, hepatocellular carcinoma, spontaneous bacterial peritonitis ).
All the patients included in our study were subjected to history taking and full clinical examination, routine
laboratory investigation included liver profile. Also, ascitic fluid analysis was done as regard chemistry ,
bacteriological and cytological examination. At last, the alpha fetoprotein and spiral CT scan were done This study concluded that there was no statistical difference between the 3 groups regarding the serum
ascitic albumin gradient and also, there was no value of using the SAAG in predication of the hepatocellular
carcinoma.
Also, This work concluded that the ascitic fluid triglycerides can not be used as a good tumor marker to predict the development of HCC in cirrhotic patients as there was no statistical difference between the 3 groups regarding the ascitic fluid triglycerides.
