Abstract

The present work was designed to study the correlation between non-organ specific autoantibodies(SMA), IgG and AST levels in diagnosis and prognosis in children with AIH.
The present study was conducted on a total number of 75 child were included in this work. 45 child were suffering from AIH, diagnosed previously(by IAIHG scoring system) and receiving treatment ( prednisone and azathioprine) for a period ranging from six months to two years, were selected for this study, and 30 healthy child were chosen during the same period of time as a control group.
All patients and controls were subjected to the following:-
History taking, thorough clinical examination, complete blood count (CBC), liver function tests (bilirubin, ALT, AST, serum proteins and albumin, prothrombin time and concertration) and immunoglobulins .
All patients and controls were tested for the smooth muscle antibodies (SMA).
In our study, all liver function tests were impaired in our patients before treatment.
In our study, all patients were tested for SMA before and after treatment. SMA were elevated in AIH patients before treatment and decrease after treatment, with statistically significant difference before and after treatment.
In our study, all patients are tested for IgG before and after treatment. IgG were elevated in most AIH patients before treatment and decreased after treatment .
We noted that a positive correlation between duration of therapy and autoantibodies, with longer duration of therapy (more than 18 m.) SMA were decreased.
We found that a positive correlation between duration of therapy and liver function tests (ALT &AST) and IgG levels. With longer duration of therapy (more than 18 m) they were decreased. With immunosuppressive therapy (less than 18 m.) there was equvocal response manifested by high autoantibodies (SMA), liver function tests(ALT & AST) and IgG levels.
There was positive correlation between SMA titer, IgG level and liver function tests ( ALT& AST. where SMA is positive in cases with high liver function tests ( ALT & AST) & high IgG levels , it was negative in cases with low liver function tests (ALT & AST) & low IgG levels, with statistically significant difference between results in patients before and after treatment.
This results indicate that measurment of SMA or measurment of AST and IgG levels may be used to monitor disease activity in children with AIH.
