Abstract

Labour has been termed the most dangerous journey ever human undertakes. Although it’s a natural process it’s complications can arise atv any time during its course. A considerable proportion of the increased cesarean delivery rate has been in the number of cesarean deliveries performed on nulliparous woman for dystocia and as repeat cesarean deliveries in multiparous woman Any intervention aimed at reducing the cesarean delivery on nulliparous woman will inturn lead to reduction in cesarean delivery rate. The partogram is a tool that can be used to assess the progress of labour and to identify when intervention is necessary. This study aimed for studing the effect of use of two-hour and four-hour partogram action lines on maternal and neonatal out comes. It included one hundered primigravidae with live singleton, cephalic presentation, admitted at term in spontenous labours without maternal medical complications or fetal anomalies. The women were divided randomly into two groups:-Group A:- 50 women for whom four-hour action line partogram was used Group B:- 50 women for whom two-hour action line partogram was used No intervention was done until the patients reached the action line. If cervical dilatation fell on or crossed the action line augmentation was done by oxytocin alone when membranes are ruptured or amniotomy followed by oxytocin in the presence of intact membranes. Analysis of the results revealed that the use of two-hour action line in stead of fourhour action line increased unnecessary interventions without affecting mode of delivery, neonatal or maternal out comes.
