Abstract

Renal osteodystrophy is a spectrum of bone mineral changes that could range from the high-turnover lesions of secondary hyperparathyroidism to the low-turnover lesions of adynamic bone disease ROD is considered one of the main reasons for the underdevelopment of children with CRF. Clinical symptoms of ROD are not usually present until advanced ROD is manifested, Hence, early and accurate assessment of the skeletal system in children with CRF is essential to prevent the irreversible disability that may result from delayed treatment The goal of therapy in childhood chronic kidney disease-mineral and bone (CKD-MBD) is to normalize mineral metabolism with the aim to improve growth and reduce bone deformities and fragility and minimize the progression of extra-skeletal calcification. Biochemical markers of serum calcium, phosphorus, and PTH are primarily used to guide therapy The current study was done to evaluate the bone turnover, estimated by the measurement of serum markers of bone formation, bone resorption, and with bone mineral density in children with chronic renal failure either on conservative therapy or on regular hemodialysis The present study was carried out on 55 children divided into 3 groups Group I (dialysis group) : Includes 20 children with ESRF on regular hemodialysis. This group was divided into three subgroups according to iPTH values .Group IA (low serum PTH group IB (normal serum PTH) and group IC (high serum PTH).
