Abstract

This study was carried out on 71 subjects who were classified into three main groups ; namely, CHD, hypertension and control group. The IHD group showed a significant increase in triglyceride levels, WBC, NEUT, PF4 and PAI-1 and highly significant decrease in HDL-C and HDL- C% of total cholesterol, mono and TAT. No significant difference was found between IHD and control group as regards total cholesterol, LDL- C, platelet aggregation tests, MPV, PDW, P-LCR and FPA. PF4 showed a significant positive correlation with WBC and NEUT in IHD patients. FPA showed a significant positive correlation with each of WBC and NEUT in IHD patients. A significant positive correlation was found between TAT and PF4 in IHD group. In hypertension group, a significant increase was found in triglycerides, total cholesterol and LDL-cholesterol ; a highly significant decrease was found in HDL-C and HDL-C% of total cholesterol and a highly significant increase in LDL- C / HDL-C ratio when compared to control.
The aim of the work:
1-ldentification of hypercoagulable state in patients with chronic ischemic heart disease.
2-evaluation of the ability of some laboratory markers of coagulation activity to identify individuals destined develop clinically relevant hypercoagulable states and consequently the ability of these assays to pinpoint patients most likely to benefit from prolonged anticoagulant therapy.
3-evaluation of the state of fibrinolysis in patients with ischemic heart disease through measurement of plasminogen activator inhibitor type l (PAL-1) PLASMA LEVEL.PAI-I is an important regulator of fibrinolytic activity in human plasma
The most important finding in this study is the demonstration of increased thrombin generation in IHD patients as indicated by increased TAT levels especially in those with history of MI i.e. a significant proportion of IHD patients are actually in a prethrombotic state. On the other hand thrombin activity on fibrinogen was found to be within normal as shown by normal FPA levels i.e. increased thrombin generation is still incapable of initiating fibrin formation. However, this would not be the case it triggering event (ATHEROSCLEROTIC Plural) become available.
Platelet activity in vivo was found to be enhanced in IHD patients as indicated by increased PF4 plasma levels.
The study also revealed impairment of the fibrinolytic system in IHD patients as shown by increased PAL-I levels.
Increased neutrophils and monocytes in IHD patients even within the normal range as an accompanying phenomenon of the atherosclerotic.
