Abstract

Lactose intolerance ( Lactase deficiency , Lactase non-persistence) is the inability to digest significant amounts of lactose that is present in milk (Kerner, 1995).It is manifested clinically by abdominal bloating, cramping, distention, flatulence, and osmotic diarrhea, Symptoms of lactose intolerance resemble those of some other gastrointestinal dysfunctions such as functional bowel disorders and other maldigestions. (Birge et al., 1967).
The present study was conducted on 300 child aged 6-12 years including males and females of different primary schools in Qena governorate.
Clinical data taken from each child ’parents including name , age , sex , weight , history of diarrheal attacks especially following ingestion of milk or milk products , history of chronic illness including diabetes & family history of diabetes mellitus .
overnight fasting blood glucose level is measured for each child using portable Glucometer device then lactose load of 2g/kg and maximum 50g dissolved in a 20% water solution will be given orally to each child.
& blood glucose level is measured again after two hours to determine if this child is lactose intolerant or not .
An increase to values < 20 mg/dl has been considered as indicative of lactose intolerance whereas an increase to values >30mg/dl is considered to indicate Lactose Tolerants (Lactose tolerance test (LTT) ,Joneja ,2003).
- Statical analysis of this study revealed:- a progressive decrease in lactose absorption with age which is genetically based started by 58% by age of 6 years & reaching up to 90% by age of 12 years.
- Also this study showed that only 56.8% of lactose intolerant children have positive clinical history of abdominal pain & or abdominal distension & or diarrheal attacks following ingestion of milk or milk products .
Conclusion:
-The incidence of lactose intolerance in primary school children in Qena governorate is progressively increase with age.
- Not all lactose intolerant children have positive clinical history of abdominal pain & or abdominal distension & or diarrheal attacks following ingestion of milk or milk products as this related to many factors ,mainly , the amount of lactose ingested & the degree of activity of lactase enzyme.
